Student Registration

I give consent to Truilite Baptist Church the right to take photographs and videos of my minor child, during church/VBS activities. 

I understand that these images may be used in print publications, online publications (such as the church website and social media), and presentations for the purpose of promoting the church and its ministries. 

I agree that my child's name will not be used in connection with the photographs, and I waive any right to inspect or approve the finished product. I also understand that no fee or compensation will be payable to me or my child by reason of such use.
